In order to ensure our students’ safety at all times, please read and consent to the following specific rules that apply to the care of students under the age of 18 during their stay in Australia.

**Autralian Law**

1) “NO” Consumption of alcohol
2) “NO” Smoking including illegal substances
3) “NO” Entering licensed premises (Bars and/or night clubs)

**College Rules**

1) Curfew times (Students are to be home by 6pm Sunday to Thursday and by 8pm on Friday and Saturday)
2) The host families always need to be informed about absences during free time, or if the students is going to be late for dinner
3) A Surfing Permission Form needs to be signed by a parent if students wish to go surfing unsupervised with friends
4) Students are not allowed to go surfing unsupervised alone
5) The students are responsible for potential damages caused at the accommodation

Any violation of Australian Laws or College Rules, may result in the student being sent home at their parents’ expense before the completion of their course.

By signing this form I authorise my son/daughter to attend supervised activities which may include: surfing, kayaking, mountain biking, rock climbing, excursions, barbeques and other activities as organised by Lexis English staff and its contracted providers.

By signing this form, I authorise Lexis English to sign any waiver forms for activities which my son/daughter attends and I understand and agree that Lexis English or contracted providers with Lexis English will not be held liable in the event of any accidents.

Students participating in Lexis Skills – The Beauty House Academy Program will participate in beauty treatments as part of their program. By signing the form below parents consent to this arrangement and agree to students performing and receiving treatments taught during the program. These skills are overseen by trained professionals.

Lexis English will not be held liable in the event of any harm coming to the student(s) while outside of the provided supervision.
Furthermore, Lexis English will not be held liable for any damage or monetary liability caused due to misconduct on behalf of the student(s) while enrolled at Lexis English.

Even with the permission students are not allowed to go for unsupervised surfing alone. Lexis English operates under a strict “Buddy System” for all water-based activities.

Lexis English will not be held liable in the event of any harm coming to the student(s) while outside of the provided supervision.

Furthermore, Lexis English will not be held liable for any damage or monetary liability caused due to misconduct on behalf of the student(s) while enrolled at Lexis English, during the student’s enrolment.

I hereby agree that my son/daughter has my permission to go on the above unsupervised surfing except these prohibited surfing points.

Unsupervised Surfing Declaration(Required)
